What’s included
- Request details: project and contract number, COR number and revision, who it’s submitted to, what caused the change, the date, and the contract clause or notice reference.
- Description & reason: a box to describe what changed, where and when, and how it differs from the original scope.
- Cost breakdown: labor, materials, equipment, subcontractors and other costs with their backup, then subtotal, markup, bond and insurance, tax and the total requested.
- Schedule impact: calendar days requested, affected activities or critical path, and lead-time assumptions.
- Attachments: photos, tickets, quotes, correspondence and schedule backup.
- Status & approval: status, approved amount and days, the change order number issued, and signature lines for both sides.
How to use this template
- Fill in the COR number, the project and who it’s going to.
- Say exactly what caused the change: the RFI, directive, drawing revision or field condition, with the date.
- Describe the changed work and how it differs from your original scope.
- Break out the cost with backup for every line, and apply markup only as your contract allows.
- State the schedule impact. “To be determined” is fine, but don’t leave it blank.
- Attach your backup, send it per the contract’s change procedure, and don’t start the work until it’s authorized in writing.
